Documents Required For Registration

At the time of registration, please have the following information:

• Registration Fee
• Completed Saint Luke School application form
• School Visit (not required, but highly recommended)
• Birth Certificate
• Baptismal Record (if applicable)
• First Reconciliation Record (if applicable)
• Immunization Records:
  • DPT (Diphtheria & Tetanus) or "Baby Shots" (4 shots; the 4th must be on/after the child's 4th birthday)
  • Polio Vaccine (3 doses)
  • MMR Vaccine (Measles, Mumps, Rubella - 2 doses)
  • Varicella Vaccine (Chicken Pox - 1 dose or a signed statement that the child has already had them)
  • Hepatitis B Vaccine (3 doses)
• Other pertinent medical records
• Signed "Request For Records" form
• Reports from Auxiliary Services (e.g. Title 1, Speech Therapy, Act 89, and/or other - if applicable)
• Individualized Educational Plan/IEP (if applicable)
• Psychological Reports (if applicable)
• Behavioral Reports (if applicable)
